When you do attend the practice, there will be some new protection measures in place. We understand that some of these might seem a little strange at first so please read the following information to see how you can help us keep everyone safe.

  1. After confirming your appointment on the phone we will ask you to fill in your Consent & Medical History forms which can be found on our website. Once you submit these they will be sent directly to us.
  2. Please arrive 5 minutes early on the day of your appointment. When you arrive at the practice please wait in your car or outside the surgery and call our reception on 01780 763133 to let us know you have arrived. When Mr. Hawkins is ready for you we will ring you and ask you to come in. Please wherever possible come to the practice alone and with the minimal amount of belongings.
  3. On entry to the practice you will have your temperature checked and you will be asked to use our hand sanitiser. Please also advise us at this point if anything has changed on the recent medical history form you completed.
  4. If your temperature is over 37.8 degrees you will be asked to leave and contact 111 on your return home.
    If your temperature is under 37.8 degrees we will ask you to place your belongings in a box and you will be taken through to the surgery.
  5. Please arrive with your teeth sparkling clean as you will not be able to use our bathroom for tooth brushing.
  6. The team will be wearing more Personal Protective Equipment (PPE) than before, but don’t worry, it’s the same team underneath!
    Mr. Hawkins will proceed with your assessment and agree on the best treatment that we are able to offer at this time with you. If possible and appropriate we will carry out treatment that is agreed.
  7. Contactless payments are available at reception. For larger amounts, we will arrange payment over the telephone following your appointment.
  8. You will then be discharged from the practice ensuring minimum contact with other patients. We will telephone you after your appointment with any further advice.
